Dali and the Bridge: A Case Study in the Cost of Ignoring High Reliability Principles
On March 26, 2024, the container ship Dali struck the Francis Scott Key Bridge in Baltimore, causing a catastrophic collapse and claiming six lives. While the incident is still under formal investigation, the preliminary reports and voyage data recorder (VDR) transcripts reveal a troubling pattern: a failure to operate with the vigilance expected of a high reliability organization (HRO).
HROs, like aircraft carriers or nuclear power plants, succeed in managing risk not because they avoid danger, but because they anticipate and contain it. The Dali case reflects the opposite.
Preoccupation with Failure: Absent
HROs continuously seek out small failures as signs of deeper issues. On the Dali, multiple alarms began sounding the day before departure, some so persistent that they rendered bridge conversations unintelligible. Yet the response was disturbingly casual. The bridge was vacant for extended periods while alarms blared. At 17:49, the Master instructed the Chief Engineer to file an incident report, yet no mention of these issues surfaced during the pilot-master exchange.
How could such warnings go unheeded?
Lesson: A system cannot respond to weak signals if it never takes them seriously. Dali’s crew normalized deviance, resetting alarms without resolving root causes, violating the spirit of preoccupation with failure.
Recommendation: Require a “technical readiness attestation” before departure, signed by both engineering and navigation officers, confirming all alarms are resolved, not just silenced.
Reluctance to Simplify Interpretations: Ignored
As the departure neared, the bridge team engaged in light conversation. Pilots relied on general statements like “everything’s in order,” ignoring the nuance of earlier electrical faults and breaker trips.
HROs avoid oversimplified narratives. They recognize that complex systems can fail in surprising ways. The Dali crew opted for procedural normalcy over diagnostic precision.
Lesson: Simplistic assumptions about system status concealed deeper vulnerabilities in the vessel’s electrical system, vulnerabilities that triggered two blackouts en route to the bridge.
Recommendation: Mandate a pre-departure fault status review with detailed explanations of all recent system anomalies logged and briefed.
Sensitivity to Operations: Delayed
The VDR shows that critical decisions, calling for tugs, dropping anchor, notifying authorities, were made within minutes, or seconds, of the allision. Despite an obvious power loss, steering and propulsion were not regained in time.
HROs are marked by deep operational awareness, what’s happening, what could go wrong, and how quickly things may change. Here, delays and indecision hinted at a crew unprepared to respond quickly to cascading system failures.
Lesson: Without real-time situational awareness and drill-honed response plans, even seasoned mariners may freeze under pressure.
Recommendation: Institute mandatory emergency response drills focused on loss-of-steering and blackout scenarios while underway, especially in confined or critical waters.
A Broader Message
The Dali tragedy was not just a technical failure. It was a systemic organizational failure to embody high reliability. From complacency in the face of alarms to vague handoffs and last-minute reactions, the conditions for failure were in place long before the ship reached the Key Bridge.
Maritime operators must internalize the lessons of HROs: question everything, track every anomaly, and drill for the worst. Because in high-stakes environments, reliability isn’t a feature, it’s a discipline.
